Since our opening nearly 30 years ago in Hendricks County, our staff has been providing safe, comfortable, and excellent nursing care for the families in Avon, College Street, Mooresville and Indianapolis. Our residents receive comprehensive services customized to their unique needs and challenges in our Rehabilitation and Skilled Nursing units.

Caring is our Business, that has been the mission of College Street Health Care Center since our opening. Our staff is committed to providing excellent nursing care and rehabilitation for families. Our compassionate approach to meeting the needs of our residents improves their quality of life and gives their families peace of mind.

Our recently opened Memory Care neighborhood called “Caring Hands” provides all the comforts of home in a secure environment. We know that each guest has specific individual needs – our goal is to meet those needs with a customized, positive approach.
